Replacement Certificates

This certificate you receive upon successfully passing the written and practicum exam is a legal document which shows you are trained to perform Spirometry testing.  We suggest you make a copy for your files and frame the original for hanging in your spirometry station area.

If you misplace the certificate,  you need not call our office.  Please fill out this form and mail it to the address below with a check for $25 made payable to Palmer Associates.
